If you tighten it too much you can flatten the umbrella shaft. The light then shoots into the open umbrella. Think of it like a satellite dish, the flash fires into the dish and then is reflected back past the flash. With a Shoot-thru umbrella you point the umbrella at the subject instead of having it reflect back past the flash.
